I feel as though I've reached a plateau in growing my hair out. I am easily at nipple-length now, but it seems as though any of my other goals (waist-length and longer) are so far off in the future that it really doesn't do any good thinking about them.
I'm worried that I might start getting apathetic in caring for my hair, which will result in it not being in the shape it needs to be in for it to look good long.
What has everybody else done to keep motivated after they reach a certain, intermin length?

Only advice I can give is to stop thinking about the hair. Dont neglect it but dont have it on your mind all the time. I found that my hair grew faster this way from mid back to waist length. I know its easier said than done but it works.
Also try to enjoy where youre at, I found that after I reached my goal I got bored of my hair quickly. It seemed much more fun growing it out and watching the changes it goes through, but that could just be me.

Jon, I am no where near your length and never have been. I would suggest, perhaps, that you try to find some other exciting things to do with your hair while it is simply growing. Why not try to research and wear some traditional hair styles from other periods or cultures that long hair men used to wear (like Japanese top knots, or Celtic braiding). That might give you some focus for your hair and developing a skill of perfecting that application of these techniches.
