I am 38 years old I have had long hair off and on my entire life. MY curent long hair I have had for 8 years. Resently I was in the hospital for a week. Since it was surgery no shower either. When I got home my hair was one big knot to say the least. So I had my wife brush it out. Was that a mistake? Because it ripped lots of hair out. Now its not noticable to all but it feels differnt to me. It feels thiner and you can see the line in back of my dark hair. People have to shave my head grow it back, others say to cut it short and grow it back. some say, cut to the back to where the ends broke, some say just grow it out. I neen advise what to do.
Go to a salon where all can be attacked by people of experience with this sort of thing. They will use combinations of hands and combs, and even for them it is quite tough........very! You WILL lose alot of hair, but if your wife or yourself attemps a task such as severe as is this, chances are high that you will lose much more.
When finished you will still have long hair, and much to your surprise will have more of it than you thought possible and it will probably even look well despite not having the thickness it once had. (But that will all grow back in time.)
I would not have it cut however except for a small "trim" of the ends after all has been unknotted and a comb will once again go through it real easy.

My eyes were going faster than your words in the above response. Apologies. So, your wife HAS already done all. Bet she was exhausted. Whew! In this case, just let all grow back but no need to cut. In the end all will be looking as you were so used to before. :-)
I'd just let it grow. If it is not noticeable to a lot of people, as it grows it will just get better and better, and noticeable by less and less people. Taking hair off WILL be noticed by EVERYONE, and your hair will look shorter to them during the growing out period. If you "do nothing" it will perhaps look thinner, but it will not look shorter, and of course the physical reality is that you will have MORE HAIR."

Well, without a picture to look at, it's hard for any of us here to give you our best advise, as we will only be guessing (based on an imaginary picture of how bad or OK it is)... In any case, though, I am certainly sorry to hear of the ordeal that you've recently been through.
Also, another comment re. advise in general: only YOU can ultimately decide what is best in your own eyes. Some guys here, for example, like to have a very professionally-trimmed look to their hair (for example, guys who like to trim their hair straight across at the bottom, giving a look of all one length), --- while other guys like the look of a more natural V-shape at the bottom (which Nature itself more or less accomplishes without any cutting having to happen). Long hair naturally tapers off and thins out at the ends around terminal length, anyway, --- so, your decision all depends upon how long you wish your hair to ultimately become.
