Paint comes out very easily under water when wet... so I doubt that there are any lasting effects lol
I would be less concerned about their hair (I assume they are using water-soluble acrylic paint) than what they do to their brains and spinal cords with all that shaking around!

Their brains may already be beyond saving.
I have no experience of the Heavy Metal world, but I looked it up on the internet. Head shaking and violent movements are parts of what they do. Medically, whip-lashing one's head as violently as that can cause traumas like in car accidents. The neck is about the most fragile part of us. One Metal group leader had a stroke because of the head shaking, but he fortunately recovered. He had to learn to do things in a new way without violent head shaking.
People are free to do what they want, but need also to know the risks. It's a little bit like extreme sports.
